Ketakutan terhadap Kematian sebagai Manifestasi Gangguan Cemas Menyeluruh pada Lanjut Usia di Masa Pandemi COVID-19: Laporan Kasus

Gangguan Cemas Menyeluruh (GAD) pada lanjut usia sering kurang terdiagnosis akibat tumpang tindih gejala dengan proses penuaan normal, terutama diperburuk stresor pandemi COVID-19 seperti paparan berita kematian berkelanjutan. Laporan kasus ini bertujuan menggambarkan manifestasi klinis, proses diagnostik, dan penatalaksanaan biopsikososial GAD yang dipicu kecemasan kematian terkait pandemi pada pasien wanita berusia 60 tahun. Menggunakan pendekatan deskriptif observasional kualitatif di Poli Psikiatri RS Muhammadiyah Lamongan, sampel purposif tunggal memenuhi kriteria inklusi cemas berlebih ≥6 bulan dengan onset pandemi tanpa komorbiditas kognitif/medis. Instrumen mencakup auto/heteroanamnesis, pemeriksaan status psikiatri komprehensif, skala GAF (skor 51-60), dan kriteria DSM-5-TR/PPDGJ III, dianalisis melalui triangulasi sumber dan analisis konten tematik. Hasil menunjukkan ketakutan kematian harian sulit dikendalikan, gejala somatik (gemetar, gelisah), insomnia dengan kognisi terjaga. Penatalaksanaan terintegrasi meliputi sertraline 50 mg/hari, trazodone dosis rendah, psikoterapi suportif, dan psikoedukasi keluarga efektif mengurangi gejala. Temuan ini menggarisbawahi stresor pandemi sebagai pemicu utama GAD pada lansia dan urgensi skrining dini.
